New Patient Registration
If you live within our practice area you are welcome to register with us and our Reception staff will be happy to guide you through the procedure.
You can register quickly and easily online by accessing the following link:
Alternatively you can request from our Reception team a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
You will then need to take the completed forms and ID into the surgery so that your registration can be completed. Until this has been done your registration cannot be completed, therefore it is important to do this as soon as possible.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
